Job Description:
Description Job Summary: Provides nursing care as a member of
the healthcare team delivering specialized care for patients with
Renal Replacement health condition. Responsible for direct patient
care with specialized modalities to include: hemodialysis,
peritoneal dialysis, and continuous renal replacement therapies.
Applies evidence based practice in the planning and delivery of
care and conducts ongoing education to patients and families.
Supports PICU, NICU, CICU, and Medical/Surgical Adolescent Medicine
inpatient nursing units with acute services. Supports and upholds
quality standards of a Magnet designated hospital and Patient Bill
of Rights. Practices under the supervision of the Nursing
Manager/Director. Minimum Job Requirements: - RN - Registered Nurse
within the State of Florida - maintain active and in good standing
throughout employment. - CPR BLS - CPR American Heart Association
(AHA) - maintain active and in good standing throughout employment.
- PALS - American Heart Association (AHA) - maintain active and in
good standing throughout employment. within 2 years - 1-3 years RN
experience Required Qualifications Job Specific Duties: - Provides
nursing care and performs treatment to Renal Replacement in the MCH
outpatient center and designated inpatient nursing units. -
Delivers appropriate treatment technique such as hemodialysis,
peritoneal dialysis, and continuous renal replacement therapies. -
Completes nursing clinical assessment, obtains vitals, patient’s
weight, monitors patient responses to treatment therapy, and
adjusts and modifies treatment as directed by physician. - Performs
all required pre-treatment dialysis machine alarm testing,
including Pressure Holding Test (PHT) and sets up dialysis machines
in preparation of patient’s treatment. - Ensures safe and clean
working environment, cleans and disinfects dialysis machine,
performs water testing, and machine calibration in accordance with
department policies and procedures. - Implements, administers, and
monitors patient's response to prescribed intradialytic
transfusions; escalates and notifies the physician of any adverse
reaction. - Collaborates and communicates with physicians and other
members of the healthcare team to interpret, adjust, and coordinate
daily patient care plan to ensure continuity of care. -
Collaborates with the inpatient nursing unit, clinical, and medical
teams, and delivers appropriate treatment technique using portable
equipment and machines. - Communicates the patient's progress
including critical findings and changes in the patients' condition
with the physician and/or disciplines involved in the care of the
child. - Reviews patient admission history, prepares and completes
the discharge planning process accurately as per department and
hospital policies and procedures. - Follows physician’s orders,
administers and reconstitutes medications directed. Adheres to the
six rights of medication administration. - Collaborates and rounds
with a multi-disciplinary team in daily care planning rounds, as
appropriate and fosters a Family Centered Care environment. -
Accesses electronic health record independently according to
hospital policy and completes accurate and timely documentation in
the patient’s electronic health record. - Communicates with
patients and families to review plan of care and solicits input.
Communicates any patient and family concerns to physician. - Assess
patient and family understanding of therapy regimen and conducts
ongoing education for patients and their families. - Educates
patients/family members on renal replacement therapies and post
treatment home care such as disease, medication, treatments, pain,
discharge, and safety. - Promotes an /"error free/" work
environment, consistently provides high-quality, safe care as per
hospital policies, conducts prevention rounding, and promptly
follows up on concerns. - Assesses all body systems, ensures safe,
effective, efficient care delivery with emphasis on the absence of
medication errors, falls avoidance, ouch-free procedural pain and
reduction of pain. - Provides patient information and effectively
completes the hand off process at shift change and patient
reassignments. Knowledge/Skills/Abilities: - Strong problem
solving, critical thinking and conflict resolution skills. -
Ability to manage multiple priorities and strong time management
skills. - Effective verbal and written communication skills. -
Computer literate. - Career Development: National certified
preferably within 5 years of hire. - BSN Degree or currently
enrolled in BSN program. - Nursing experience in a Pediatric
healthcare setting is preferred. - Certification in Nephrology
Nursing is preferred. - Works effectively in a collaborative
environment. - Ability to apply principles of evidenced-based
practice, analytical thinking and service excellence in practice. -
Analyze and interpret data and utilize the information to make
judgments regarding patient care. - On-call assignment per
